Need Statement for this Activity:
Mortality rates remain high for Heart Failure patients, while CAD is a leading cause of death, and there is an educational gap on the diagnosis, assessment, and management of patients with Atrial Fibrillation. In addition, Pulmonary Heart Disease is often quite difficult to treat and the Hospital 2 Home initiative to improve the transition from inpatient to outpatient status is an important project for those with cardiovascular disease.
Overall Goal for this Activity: The overall goal of this activity is to improve patient care by increasing learner competence in treating Heart Failure, Coronary Artery Disease, Pulmonary Hypertension, and Atrial Fibrillation; while also increasing their knowledgebase on Percutaneous Valve Replacement and the improvement of overall performance in using appropriate guidelines-based treatment options.

Nurses
American College of Cardiology Foundation is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.
The ACCF designates this educational activity for a maximum of 4.25 continuing education hours. Requirements for successful completion are attendance in a presentation in its entirety and completing the evaluation tool.
While offering CME and CE credits noted above, this program is not intended to provide extensive training or certification in the field.
