NEED STATEMENT FOR THIS ACTIVITY:
Mortality rates remain high for Heart Failure patients while CAD is a leading cause of death. Valvular Heart Disease, likewise, remains prominent among cardiac patients and a considerable percentage of the population suffers from diabetes. In addition, the Quality of Care issue will continue to have a significant impact on cardiology practices and their patients.

OVERALL GOAL STATEMENT FOR THIS ACTIVITY:
To improve patient care by increasing learner competence in treating Heart Failure, Coronary Heart Disease, Valvular Heart Disease, and Diabetes, as well as improving performance in using appropriate guidelines-based treatment options.

Examine the correlation of type 2 diabetes and cardiovascular (CV) disease so that diabetic patients at risk of CV can be identified, prevalence of CV disease in such patients can be described, and modifiable and controllable risk factor strategies can be applied to achieve optimal outcomes.
